Health Care Tax Form 1095-C

Form 1095 is a tax form required under the Affordable Care Act (ACA). This form will play an important role in your annual tax filings. Get help to understand the form, why it’s important, and how it affects you.

1095-C forms are available in OneUSG Employee Self Service for all active employees.

  1. Log into OneUSG Connect
  2. Go to "Benefits"
  3. Click the "Manage My Benefits" tile
  4. Select the "View/Print Your 1095" tile
  5. Select "View/Print Your 1095"

If employees did not consent to receive their 2022 1095-C electronically, they will receive it by mail on or before January 31.  The VSU HR office does not have access to print 1095-C copies for employees.  If you have questions or concerns, please contact the Shared Services Center at 1-877-251-2644 or
