Students with Disabilities Testing Confirmation Request

Please fill out the form below to provide approval and proctoring information for the exam. If the student arrives late for the scheduled exam time in the Access Office, are we authorized to allow the student to take the exam?
    • This will only be used on the day of the exam in case there are testing complications such as no exam submitted, incorrect password, clarification, student no show, etc.

    What time may the student begin their exam?

    If the time does not appear in the drop down box, please type it in the text box labeled "Other."

    • Please list the amount of time the class will be allowed for the exam.

    If the amount of time is not listed in the drop down box, please specify the amount of time using the "Special Instruction" box at the end of the form. 
    For continuity, please only list the amount of time the class is allowed for the exam. The Access Office will calculate additional time for accommodations. This is intended to prevent any confusion or miscommunication.

    • PLEASE DO NOT PUT EXAMS or MATERIALS in INTERCAMPUS MAIL. You may pick up/deliever exams and materials as needed. Attached exams will be printed for the student. Once completed exams are returned to the professor, it will be shredded.
