This checklist must be completed each semester that you wish to use VA benefits. Please only submit this checklist, after enrolling in classes for the semester.

    • An updated Certificate of Eligibility is needed every semester to process Chapter 33 (Post 9/11) and Chapter 31 (VocRehab) benefits.

    A Certificate of Eligibility for other chapters is typically only required the first semester of attendance.
