CCAF/JST EVALULATION FORM INSTRUCTIONS

Please provide the following information:

• Full Name (Last, First) 
• VSU ID (For current students) 
• Academic Advisor's Name (For current students) 
• Program of Study 
• Branch of Service  
• Current Status - Active Duty/National Guard/Reserves/Retired/Separated

Attachments:

• Community College of the Air Force (CCAF) / Joint Service Transcript (JST) 
  • Official or Unofficial are acceptable
  • If the file is larger than 2MB, please send the file to amp@valdosta.edu with your name and confirmation that you have already submitted an evaluation application.
