Procedure:  An employee may request an accommodation by completing the forms under ADA Procedures. The forms must be returned to Human Resources as soon as possible.  Upon receiving the forms, Human Resources will then contact the appropriate dean/director and work through the process.  Human Resources and Employee Development will serve in a consulting role for all employment related issues and the Access Office will serve as a consultant for material and resource related information.  Resolution will involve all parties, with Human Resources guiding the process.
