Registration Appeal Form:

This form is used along with the Late Registration and Drop/Add form for any registration activity (including new registration and drop/add) following the deadline as published in the Registration Guide. A LATE FEE applies for new registration.

  1. Pick up the registration appeal form and late registration and drop/add form from either the Registrar’s Office or the Student Advising Center.
  2. Complete the forms.
  3. Obtain Instructors’, Department head of major, Dean of major, and Vice President for Academic Affairs
  4. Return to the Registrar.
This form is not official until processed by Registrar’s Office personnel.

A $75 late fee applies during late registration.
