Youth.net  has many mini-lessons for language arts.  These lessons are for k-5 grade levels.  It also contains links to many other helpful websites.

Reading
The Learning Train offers articles, resources, and references for the teachers.  This site offers activities for the students to help with word recognition.

The Funschool is a website that lives up to it's name.  It has many fun activities for all grade levels.  This site offers games, printables, and downloads.  This site also offers connections to the Teacher's Basement.
Math
Math.about.com is a website that furnishes many activities on measurements, shapes, numbers, opposites, patterns, and colors.  It gives ideas for teaching these kinds of lessons.

The Math Forum focuses on activities for learning numbers and operations, measurements, geometry, and other math concepts.  This site also have lesson plans for teachers.
Social Studies
America's Story allows students to learn the history  of America through activities and games.

The Center for Civic Education is a website full of sample lesson plans for teaching civics to the students.  These lesson plans range from the history of our country to the government of our country.
Science
Exploratorium is a website that has hands-on-activities for students.  This site also offers teaching techniques for teachers.

Thursday's Classroom is a website that features science stories for students and lesson plans for the teacher.  This site also has news articles about modern science.

Music
The Children's Music Web is non-profit resource center for teachers, students and parents

M.U.S.I.C. is a website that offers a reading room, classroom activities, and stories behind the songs.  This site also offers a collection of over 3000 songs relevant to education.
Physical Education
Fitness for Youth is a website for parents and teachers. This site has many games to help in the development of fine motor skills.
PE Central offers lesson plans and assessment ideas for teachers.  This site also offers ideas for a successful field day.

Health and Hygiene
A to Z Teacher Stuff is a website that offers a variety of lesson plans for the teaching of health and hygiene.  These lessons ranging from dental to healthy eating.

Keep Kids Healthy is a website full of good information for parents and teachers.  This site contains information on just about all health related questions.  There are activities for students to help teach them about good hygiene.
Art
