1. Complete Student Teacher Observation Information Forms

   A. Complete Student Teacher Supervisor Observation Forms for each of the three lessons that will be observed by your supervisor and your three self-evaluation lessons.  The forms will be in your LiveText account under Forms (on left side column).  There will be a form for each of the three lessons observed by your supervisor and your three self-evaluation lessons.  Since the forms ask questions about the lesson you are going to teach, you should complete each form just before you teach your lessons.


2. Student Teachers: Download Lesson Plan Template for Attached Lesson Plan then Attach Lesson Plan and Submit to Supervisor

   A. Download the Lesson Plan Template for Attached Lesson Plan, one at a time for each of the three observations by your supervisor.   Names for each of the Lesson Plan Template for Attached Lesson Plan:
     1st (First) Lesson Observed by Supervisor,
     2nd (Mid-term)
Lesson Observed by Supervisor,
     3rd (Final)
Lesson Observed by Supervisor

3. Student Teacher Self-evaluation Using the Teacher Observation Instrument

   A. Download the Lesson Plan Template for Attached Lesson Plan.  Names for each of the Lesson Plan Template for Attached Lesson Plan:
1st (First) Self-evaluation Lesson
2nd (Mid-term)
Self-evaluation Lesson
3rd (Final)
Self-evaluation Lesson

   B. Attach your lesson plan.  (Directions below, How To Download “Lesson Plan Template for Attached Lesson Plan” Template & How to Attach Lesson Plan)

   C. Share your Lesson Plan Template for Attached Lesson Plan document with yourself.  When you look up name for sharing for your self-evaluation, you will look up your own name to share. (Directions below, How To Submit Your Documents for Review)

  
D. Complete self-evaluation of your lesson using the COE Observation Instrument (ST Self-Eval. Rubric)" from your LiveText Inbox.  (Directions below, How To Use the “COE Observation Instrument (ST Self-Eval. Rubric)"  to Evaluate Your Self-evaluation Lesson).


4. How To Download “Lesson Plan Template for Attached Lesson Plan” Template & How to Attach Lesson Plan

   A. How to Download Template Directions
1. Login LiveText at: college.livetext.com/college.  (Using FireFox, Internet Explorer, or Mozilla)
2. On your main page, My Desk, select Create button (top of page).  The Create Document page opens.
3. From Choose a Folder dropdown list, under
Valdosta State University select Lesson Plans.
4. From the Choose a Template dropdown list, select Lesson Plan Template for ATTACHED Lesson Plan.

5. Wait until the screen displays the
template outline on the right side of the screen
6. Enter the Title for your lesson plan template attached lesson plan.  You will need a total of three templates for your supervisor and three for your self-evaluations.  You should download one template at time as needed, first, mid-term, and final.  Below are the names for each of the Lesson Plan Template for each of the Attached Lesson Plans.

         a.Names for title of lessons observed by supervisor:
              1st (First) Lesson Observed by Supervisor
              2nd (Mid-term)
Lesson Observed by Supervisor
              3rd (Final)
Lesson Observed by Supervisor
          b. Names for titles of self-evaluated lessons
              1st Lesson  for Self-evaluation Student Teacher Observation
               2nd Lesson  for Self-evaluation Student Teacher Observation
               3rd Lesson for Self-evaluation Student Teacher Observation
     
9. Click “Create Document” button.  (see below How to Attach Lesson Plan)

  B. How to Attach Lesson Plan

   1. After Lesson Plan Template for ATTACHED Lesson Plan opens.
   2. Select “edit” for the Section for Attaching Lesson Plan.
   3. Complete the sentence in this section: Attached below is a lesson plan written for . . …
   4. To add lesson plan file, click on “edit” at the end of the Attachments.
   5. An attachment window opens.  To add your lesson plan file attachment, click on “Browse” and select your lesson plan to attach and click on Open.  Then the attachment window will open again.  Select Attach and then Finish.

   6. This takes you back to the Edit page, and you should see your attachment at bottom of page.  Select Finish when you have completed your editing.
   7. Now you are ready to share your Attached Lesson Plan Document with your course instructor and the person who observed you teaching the lesson.

C. How To Use the “COE Observation Instrument (ST Self-Eval. Rubric)" to Evaluate Your Self-evaluation Lesson
   1. Open LiveText at http://college.livetext.com/college (Using Internet Explorer or Mozilla).
   2. To login enter: your username and password.
   3. On My Desk (main page), click on "Reviews" (under Collaborations on left side).  
   4. Under "Received for Review" tab, click on “Review” on the right side of the “Lesson Plan Template for Attached Lesson Planthat you sent to yourself.
   5. The “Lesson Plan Template for Attached Lesson Plan” document will open.
   6. You can use a rubric for your self-evaluation.  Select "Assess" at top right of page.
   7. After the “Assessment Chooser” window opens, select the rubric to use.  Carefully select the rubric is
“COE Observation Instrument (ST Self-Eval. Rubric)," which is located in your Inbox.
   8. Click on “Select” on the right side of the rubric, 
“COE Observation Instrument (ST Self-Eval. Rubric).  The rubric window will open to evaluate document to select levels and to write comments. 
   9. After you have evaluated the 
“COE Observation Instrument (ST Self-Eval. Rubric)," then next to "Reporting," select "Official."
  10. Select the appropriate "Milestones" level.

  11. When you have completed the evaluation, click on "Save & Submit Assessment."  (If you have not completed the evaluation see note below.)

Please note if you have not completed the evaluation:

 (If you have not completed the evaluation and would like to save what you have done to work on later, next to "Reporting," select "Practice," and then select “Save”.  When you open this rubric again to complete the evaluation, your previous work and comments should be there and ready for you to continue.  When you have completed the evaluation, see steps 9, 10 & 11 above.) 


D. How To Submit Your Documents for Review
  
1. On My Desk (main page), find your document you would like to submit for review.
   2. To open: Click on your document for review. 
   3. After your document opens, click on the "Submit for Review" top right of page.
   4. The Send to Reviewers window will open.  Type reviewer's name or username..
   5. Click on the name of your reviewer. 
   6. Click the on the "Send" button.

E. How To View the Assessment for Document that You Sent for Review
  
 1. Open LiveText at http://college.livetext.com/college (Using Internet Explorer or Mozilla).
   2. To login enter: your username and password.
  
3. On My Desk (main page), click on "Reviews" (under Collaborations on left side).  
   4.
Select "Sent for Review" tab at top of page.  Click on "View" next to the document that you would like to view your assessment.
   5. After your document opens, click on "View Assessment" at top of page.  Assessment window will open and select the assessment rubric for your document.
   6. A rubric window will open to view the assessment for your document.  When you are done viewing your assessment, you can select "Close" at the bottom of the assessment window. 
(If you are having problems with opening the rubric window or opening a rubric, the problem may be the pop-up window setting.   See information below about pop-up blockers.)



F. When Evaluating a Document and the Assessment Window Does Not Open because of Your "Pop-up Blocker"

1. Open Internet Explorer and click on Tools on the top toolbar.  Select Pop-up Blocker and a side tab will open.  The side tab should have "Turn On Pop-up Blocker" showing.  See illustration below:

   * (If side tab has "Turn Off Pop-up Blocker" then select "Turn Off Pop-up Blocker."  This will change your side tab to "Turn On Pop-up Blocker" when you open the tab the next time and will allow your assessment window to open.)
