OPT Application Instructions

1. Read ALL of the information available.
2. Fill out application for Employment Authorization (Form I-765). Use an address that you know you will be at when the EAD (Employment Authorization Document) is to arrive. We recommend that you use our address, which is Valdosta State University, Center for International Programs, Valdosta GA 31698. You will be contacted when your card arrives.
3. Determine the start date of your OPT.
4. Set up an appointment with your International student advisor. Bring all the necessary application materials to the meeting. Your new I-20 with OPT recommendation will be issued then.
5. CIP will mail your application to the USCIS

OPT application are available at the Center for International Programs or you can print it out from the CIP webpage.
