Program of Study Forms

A program of study is your record of the courses that you plan to take to fulfill graduation requirements and meet your career goals. It must be developed jointly by you and your advisor. Ask your advisor for help in selecting the appropriate form from the list below.

Graduation Application

  1. Fill out these forms:
  2. Sign all forms in appropriate places.
  3. Write a check for $25, made out to Valdosta State University.
  4. Write a memo for the Bursary that includes
    • Your student ID number (the one that starts with 870)
    • Your expected semester of graduation
    • Your advisor's full name
    • Request that your receipt and forms be sent to your advisor, c/o MLIS Program, Odum Library.
  5. Send your forms, memo, and check to:
    • Bursary, Valdosta State University, 1200 N. Patterson Street, Valdosta, GA 31698
  6. Send your advisor an e-mail that includes the date your mailed your packet.
