Instruction Scheduling Request Form

Form Purpose: This form is intended for faculty and staff of Valdosta State University to use to schedule a library instruction session. Classes should be scheduled with at least two weeks' notice, when possible. The librarian will then contact you to plan and coordinate the session. Library instruction is most effective when course instructors attend instruction sessions with their classes. Please see our guide to "Customized Library Instruction" for more information.
