Gaming Device Approval Form

In order for you to connect a gaming device to Hallnet, the device must first be approved. Please complete and submit this form to apply for approval. All fields are required. Once you have submitted the request, a Network Services representative will contact you through your VSU e-mail address within 48 hours to let you know if your application was approved or denied.

Note: If you do not know the MAC address of your gaming device, please consult the user manual for the device or contact the manufacturer of the device for instructions on how to locate it.
