Graduate assistant EMPLOYMENT PACKET
Forms needed for payroll purposes

If you have been hired as a graduate or teaching assistant at Valdosta State, the following forms will be necessary to submit for payroll purposes. Original forms must be submitted (no faxes or scanned PDF files) to the Graduate School, attention Ms. Williams.

Not included - orange employment form and appointment form. You will receive these forms separately if you are requested to attend a new GA orientation or if you meet with the GA coorindator, Ms. Williams.
Required Documents:
- Confidentiality Statement
- Right-to-Know Training
- Federal Form W-4
- Georgia Form G-4
- I-9 (Employment Eligibity Verification Form)
- Employment Security Questionnaire
- VSU Drug Free Policy Acknowledgement
- Direct Deposit Enrollment Form (PDF Format) or Cash Pay Enrollment Form (PDF format)
- Background Check Form
- Federal Form 8233 - To be completed by Non-Resident Aliens when applicable.
- VSU Tax Information Form for International Students (International students only)
Your Paperwork Checklist
- Orange GA Employment Form (supplied by GA coordiantor; signed by supervisor)
- Confidentiality Statement (signed by GA and supervisor)
- Right to Know Training Awareness Certificate (print after completing on-line training)
- State of Georgia Tax Form
- Federal Tax Form (W-4)
- Form I-9 (w/copies of two forms of ID – for example, driver’s license & social security card)
- Employment Security Questionnaire (fill in first page; sign two places on back of page; notarize)
- VSU Drug Free Work Place Policy (sign and return last page to Teresa)
- Authorization Agreement for Direct Deposit (attach voided check)
- Background Check Form
- Graduate Assistant Appointment Form (supplied by GA coordinator)
All paperwork must be completed and submitted to payroll by their assigned deadlines for set up purposes. Incomplete forms or missing items will result in delay of pay.

Note: New Graduate Assistants cannot begin work until they have cleared background checks so paperwork for payroll purposes should be completed in ample time to clear background checks (preferably before December 18 for spring 2010).
