INSTRUCTIONS FOR COMPLETING YOUR 2009 FACULTY & STAFF CAMPAIGN PLEDGE FORM
Thanks for supporting the 2009 Faculty & Staff Campaign. Your gift helps us achieve excellence in the area(s) you support. Many of our academic, arts and athletic programs depend on private funding and are able to accomplish MORE because of you. Please follow these instructions to complete your pledge form.
- Print both pages of the 2009 Faculty & Staff Campaign Pledge Form.
- Legibly print your name, address, social security number, division, department/office, work phone, home phone and email address.
- Identify the Fund/Project you wish to give to.
- Select additional opportunities to make VSU More by participating in College of the Arts or Athletics programs. Valdosta Symphony Orchestra seating choices will be made on an “order card” provided by the Symphony.
- Indicate the total amount to be pledged to VSU Foundation, Inc.
- Please indicate how you wish to pay for your pledge. Here are your choices:
Payroll Deduction: The first deduction will be made in January 2009. The total amount of your pledge will be deducted each pay period until your pledge is paid in full. Please be sure to indicate how you are paid as this is how we will calculate your deduction each pay period. We can only deduct your pledge evenly through your pay periods. The minimum deduction per pay period is $2.50 for those paid bi-weekly and $5 for those paid monthly. Please sign and date if selecting the payroll deduction method.
Credit Card: We accept VISA, MASTERCARD and AMERICAN EXPRESS. We need the following information in order to process your gift: your card number, expiration date, security number and cardholder’s name (if other than your own). We will charge your credit card in January 2009 unless you indicate otherwise. Please sign and date if selecting the credit card method.
One Time Check or Cash Gift: You may make a one-time gift by cash or check (Payable to VSU Foundation).
Online: You may go to www.valdosta.edu/give/ to make your gift. Click Give Online. Please be sure to write your designated fund(s) in the comments area. - Please do not hesitate to attach additional instructions or comments to clarify your wishes. Our desire is to record your gift properly.
