_____ Completed application form must be signed by department chair and dean of your college. Please be sure to include dates of travel and location
_____ Include current copy of your brief resume, maximum 3 pages.
_____ When applying for the Professional Presentation category appropriate documentation (letter of acceptance, a copy of the program, etc.) must be attached to the proposal.
REMEMBER: In the case of a co-authored presentation, only one award can be made.
