Single Reservation Request Form
This form is only used for a single event and multi-rooms on a particular day. You will only need one date. Please give us an additional date in case we cannot accommodate your first choice.
Repetitive Reservation Request Form
This form is to be used only if you are requesting the same time and same setup for a series of dates.
Outdoor Reservation Request Form
This form is to be used for outdoor events held on the VSU Campus.
Conference Reservation Request Form
This form is to be used when requesting space for single events requiring multiple dates and/ or rooms.
Application for Unaffiliated Use of VSU Facilities
This facility request form is to be used for off campus users.
