To complete form A, click on "complete form A" which is below. A screen will pop up and you will be given the option to save to a disk. Make sure you click this. Save form A to your floppy drive A:\. Form A will be saved to your flash drive as FormA.doc. You will need to open this document in word to complete it. If you do not have Word you can open Form A as a webpage here.

You will not be assigned a Field Placement UNTIL PAPERWORK IS RETURNED. A copy is sent to the principal for placement.
If I have not received your paperwork by the end of advising, YOUR PLACEMENT WILL BE DELAYED. Therefore, you will be placed wherever there are openings and your preference rankings will not be considered.
NOTE: You must complete Form B with your advisor. This form is kept in the departmental office.
