Employment Packet

Instructions

  1. Print each form and complete in its entirety.

  2. To submit paperwork and attend a brief  orientation, make an appointment with the Graduate Assistant Coordinator by emailing sojordan@valdosta.edu.

  3. There are two forms which are not included in this packet which you will receive at the appointment with the GA Coordinator.

  4. Please Note: All paperwork must be completed and submitted to payroll by their assigned deadlines for set up purposes. Incomplete forms or missing items will result in delay of pay.

Background Check Form
You must pass a background check before beginning work.

Confidentiality Statement
Must be signed by your supervisor

Right-to-Know Training
Print certificate of completion at the end.

I-9 Employment Eligibility Verification Form
Two forms of ID such as a passport, Driver's License, or Social Security Card are required.

Employment Security Questionnaire

Students must have page 2 notarized (two places)!!

 

International Students

Contact Ms. Shana Yorkey in Financial Services at 229-333-5708 regarding tax documents BEFORE making an appointment with the graduate assistant coordinator!
